Request a DEC-1 (Pink Card)

 

Please read over the information below prior to requesting a DEC-1 Card:

1.  Partners for Safe Teen Driving Presentation:

If you are still a current 10th grade student at Westfield High School, but you did not complete the Partners for Safe Teen Driving Presentation, you will need to complete that before you can be issued the Pink Card.  For PSTD presentations offered at Westfield during the school year, please see the school webpage.

If you are a current 11th or 12th grade student at Westfield High school, you will need to register and pay for the presentation using this link.

For presentations offered during the summer, please use this link.

After July 1, 2021, If you are 18yrs or older, you are no longer required to complete the Partners for Safe Teen Driving presentation.  Please use the Google Form link at the bottom of the page to request a DEC-1 Card (Pink Card)

2.   If you have already graduated from Westfield High School but you are under the age of 18 you will still need your Pink Card to obtain your license.  

If you received your card but misplaced it, please use the link at the bottom of the page to request a DEC-1 Card (Pink Card).  NOTE- the state charges a $10 fee for replacement cards that is due upon pick up.  Cash or check (made out to Westfield High School) are the only accepted forms of payment.

If you passed the class but did not attend the Partners for Safe Teen Driving Presentation, you will need to register and complete that presentation.

Once you have completed the PSTD course, please email a copy of your certificate to Ms. Dunbar , in Sub School 3, and complete the request for a DEC-1 card (Pink Card).

3.  If you have already graduated from Westfield High School and you are 18 years of age or older,

you do not need your Pink Card to obtain your driver’s license; you can show proof of having held a learner’s permit for 60 days and complete the road skills test in order to obtain your license at the DMV.  Use this link for the documents you will need to take with you to the DMV.
